Messaging & Appointments
This is our patient portal and home to our secure messaging and appointment system. Your checklist:
You should be receiving an automated email to sign up for our members’ portal called Elation Passport.
Please register your account in Elation Passport.
Complete the Patient Intake Forms via Elation Portal.
Billing & Agreements
You will receive an automated email to complete the Patient Agreement and all other forms in a secure program called Practice Better.
Please complete all forms and agreements before your first visit
You will receive an invoice to set up payment and keep a credit card on file.
